Can I get a same-day appointment?

Depending on appointment availability and type of visit needed, the answer is YES!

Will my insurance cover my visit?

We currently accept the following: First Choice Health Plans, Regence Blue Cross Blue Shield, Any Blue Cross PPO plans, Providence, Moda, United Health, Aetna, PacificSource, OHP,  Pacific Source Medicare Advantage Plan, and Medicaid, along with many other smaller insurance companies.

All first time, new patient visits cost slightly more than established patient visits, as typical of any practice. 

 

The patient always has responsibility of covering whatever insurance does not cover. Typically, our cost will be somewhere in the range of a primary care visit or an urgent care visit depending on what level of services you need.

If your family income is 200% or less of the federal poverty income level, you may qualify for our sliding scale for any remaining balance after we bill your insurance (for commercial insurance). 

What if I don't have insurance?

We offer a discounted direct cash payment option for those without insurance. Fees for service will be dependent upon type of visit and complexity, as well as any procedures that may be needed.

Due to partial grant-funding, Hearthside is also able to provide income-based sliding scale care to those without insurance. The sliding scale is only available for those at 200% or less of the federal poverty level.

Discount labs are also available "at cost" to cash-paying patients. 

What is your service location?

Our office is located in SW Bend behind Croutons on SW Century Drive.

We also have the ability to bring our mobile immediate care services to your event, workplace or school. We can see employees & students at your location for highest convenience with our mobile care. If you have an event you would like us to attend, please contact us for more information.

Why is there a different cost for new or established patients?

 

To provide safe effective care, new patients require a comprehensive review of medical history.  This allows us to provide the best care possible for you.

What do I need to do to qualify for discount cash pricing?

 

You must agree to not submit the bill to your insurance carrier.  Hearthside Medicine Family Care also agrees to not bill your insurance.  Additionally, to qualify for discount cash pricing your bill must be paid in full 10 days from the date of service. We may also ask you to submit proof of income such as recent bank statements, tax returns, etc. if you are applying for a sliding scale service.
